Job Description
. Outpatient: * Scheduling and maintaining daily schedule. * Physician initial order or referral. * Diagnosis for therapy services including date of onset; initial evaluations and all re-evaluation in accordance to Medicare and Insurance guidelines. * Plan of treatments with MD signature pertaining to claim period. * Progress notes and attendance records pertaining to the claim period to support total minutes of actual treatments and number of minutes for each modality * Clinical progress notes. * Perform videlfluroscopic swallowing studies. 2. Hospital: * Patient education sheet. * Multidisciplinary care plan. * Education for nursing staff and PCTs. * Updated forms and policy. * Perform standardized patient testing on each patient when appropriates. * Perform videofluoroscopic swallowing studies.
Job Qualification
* Master's Degree in Speech Pathology, required. * Currently licensed in Speech Pathology in the State of New York. * Certificate of Clinical Competency from the American Speech-Language-Hearing Association, required. * Eligibility for certification in Speech Pathology by American Speech and Hearing Association.
