Pay Rate: $55-$58/hr.

Monday-Friday 8:00AM-6:00PM
Sat & Sunday 10:00AM-6:00PM

Job Responsibilities:
- The incumbent will review the completed Community Health Assessment tool in the NYS Uniform Assessment, to assist the Physician in the completion of a Physicians order
- The incumbent may follow up with individuals who have received an assessment through face-to-face communications
- Responsible for reviewing and making recommendations on consumer assessments for long term care services and supports
- Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes
- Work collaboratively to monitor and address clinical quality and gaps in care
- Provide clinical and educational assistance to support project initiatives
- Demonstrate communication skills necessary to interpret age-specific responses
Qualifications
- Bachelors Degree from an accredited nursing program, preferred.
- New York State RN License, required.
- Work experience with UAS-NY or Oasis Community Assessment tool, required
- 2+ years' experience working in-home setting within the last 10 years, required
- Work experience with the adult/geriatric populations; home care experience, experience working with individuals with physical or developmental disabilities
- Computer literacy: ability to navigate new and emerging technology; ability to perform telehealth assessment; highly developed written and oral communication skills; strong interpersonal skills; ability to work in a stressful and changing environment; and proven ability to work with community groups
Knowledge of long-term care services and programs - Ability to travel throughout the assigned region, as assigned, including travel to individuals' homes
